Approach to Care
Approach will be patient center care, team approach Consisting Physician, SW, Nurse, PT, OT, ST, RT, music therapy, CNA, Dietician, patients representative and many other, with incorporation patient and caregiver interest and goals while they are on stroke rehabilitation unit. To get maximum possible recovery and prepear them for next step of there rehabilitation /recovery and safe discharge .
