You can quickly and securely manage your health information for free with Burke Rehabilitation’s patient portal, MyChart.
MyChart Features
Our secure patient portal allows you to:
- Access medical information in English and Spanish anywhere and at anytime
- View medical records
- Manage the healthcare of a dependent or family member through Proxy Access
- Update personal information and insurance at your convenience
- View and pay bills online, including opting for paperless billing
- View lab and test results
Manage Your Outpatient Physician Appointments
Take advantage of the MyChart features to:
- Complete precheck-in registration information up to five days before an upcoming outpatient physician appointment
- Request and manage outpatient physician appointments online
- Securely video chat with your outpatient physician
Access Medical Records
You can download and save any information available within MyChart if you need to print or share.
If you need your entire medical record, please fill out the Authorization for the Release of Information form [PDF] and return it in one of the following ways:

Pay a Bill Online
You can pay your bill online through MyChart, Burke’s secure patient portal that provides you with access to your healthcare information. Whether you are at work, on the road, or at home, you can view your medical information and make outpatient doctor appointments.

How to Sign Up
During your visit, ask a staff member to give you a MyChart activation code. This code is valid for 14 days. To activate your patient portal account:
- Visit MyChart
- Click on the "Sign Up Now" link
- Enter your activation code and follow the instructions on your screen
