Your time at Burke Rehabilitation Hospital is meant to help you recover enough to return home—and many patients do. Your journey may continue with outpatient therapy, home care, or at a sub-acute rehabilitation facility.

Home & Outpatient Rehabilitation

Your social work case manager will help you arrange in-home or outpatient rehabilitation after you leave Burke Rehabilitation Hospital. You can make appointments for physical, occupational or speech therapy on our hospital campus or at one of our convenient locations throughout Westchester County and the Bronx.

Skilled Nursing Facilities

If you need to go to another facility after completing Burke’s inpatient program, ask about one of the providers in our network. We provide therapists and manage clinical services for these facilities, so you can expect the same high quality of therapeutic care you received at Burke.

Schaffer Extended Care Center (New Rochelle)

Schaffer Extended Care Center at Montefiore New Rochelle Hospital is a 150-bed facility offering short-term rehabilitation/sub-acute care and skilled long-term care. Schaffer treats neurological, orthopedic and cardiopulmonary conditions and helps patients return to their homes and families. Burke provides the management of the therapy programs at Schaffer.

Sarah Neuman – Westchester Campus (Mamaroneck)

The New Jewish Home’s Westchester Campus – Sarah Neuman offers traditional nursing home care in Small House communities. Small Houses are intimate households with private rooms, en-suite bathrooms and communal meals. Burke trains and manages the therapy team.

Physician Follow-Ups

If you need care from a physical medicine and rehabilitation doctor after leaving Burke Rehabilitation Hospital or due to a new problem, see one of our specialists in:

  • Amputations
  • Bone, joint and muscle conditions, including lower back pain
  • Brain or spinal cord injury
  • Neurological conditions, such as multiple sclerosis or Parkinson’s disease
  • Stroke

You can make appointments with a physician by:

  • Talking to your social work care manager before discharge
  • Calling 914-597-2332 after discharge
  • Going onto your patient portal, MyChart, and requesting an appointment with a doctor

We’ll work with your primary care provider and other members of your care team to help you achieve your goals.
