Social Work Care Management

Burke’s social work care management team gets involved from the day of your arrival to plan and prepare for your eventual discharge. Discharge occurs once your medical team determines that you are ready to go home or to the next step in your recovery journey.

Social work care managers assist you by:

  • Communicating with family and caregivers throughout your stay
  • Scheduling formal meetings with family if needed
  • Acting as liaisons to your insurance company case manager
  • Identifying resources that will help you as you return to the community
  • Providing emotional and social support to you and your family
  • Arranging individual tutoring for adolescent patients if necessary
